LPN Job Description

Job Summary:
The Licensed Practical Nurse work with patients who need modest oversight, including children with chronic illnesses or developmental problems, and seniors with mild to moderate health issues. During their scheduled visits, they record their patients' vital signs, check that they're taking their medications correctly, and sometimes perform other duties such as cooking or light housekeeping. They observe the patient and the patient's home for any signs that might indicate ill health or advancing dementia.
Environment:

Work may also be performed primarily indoors in a patient’s home, throughout all areas, walking on uneven surfaces, wet sidewalks, and on carpeted and/or tiled floors.  Work will be performed there routinely around other co-workers, patient’s, and guests.  Due to the nature of the business, worker may be exposed to occasional slippery floors, object on floors, chemicals, sharp objects, hazardous materials and waste (including human), blood­ borne pathogens, and communicable diseases, as well as high-stress medical and/or life-threatening situations.

Duties/Responsibilities:

  1. Helping patients bathe, get dressed, go to the bathroom, and eat
  2. Administers medications or starts intravenous fluids, noting times and amounts on patients charts.
  3. Observes patients, charting and reporting changes in conditions such as adverse reactions to medication or treatment and takes necessary actions.
  4. Answers patients calls and determines how to assist them.
  5. Measures and records patients vital signs such as height, weight, temperature, blood pressure, pulse, and respiration.
  6. Provides basic patient care or treatments such as taking temperatures or blood pressures, dressing wounds, treating bedsores, giving enemas or douches, rubbing with alcohol, massaging, or performing catheterizations.
  7. Works as part of a health care team to assess patient needs, plans and modifies care, and implements interventions.
  8. Evaluates nursing intervention outcomes, conferring with other health care team members.
  9. Assembles and uses equipment such as catheters, tracheotomy tubes, or oxygen suppliers.
  10. Records food and fluid intake and output.

Required Skills/Abilities:

  1. Must have an empathetic attitude toward the sick, elderly, handicapped, terminal, dependent or disabled patient in the home setting.
  2. Knowledge of nursing and health care terminology

Hire Requirements:

  1. Must have a valid PA LPN License
  1. Must have a negative TB test within the past 6 months
  1. Must be fully vaccinated against COVID 19
  1. Must produce a clear state background check
  1. Must be a resident of PA for the past 2 years or undergo a FBI fingerprint check
  1. Must Pass an initial competency exam

Physical Requirements:

  1. Prolonged periods of standing, bending and lifting.
  2. Must be able to lift up to 75 pounds at times. ‘
